It's just clumsy because it's so passive, nobody will really feel like you are talking to them. Just put
"If you use the gym equipment, please use a towel to lay on."

The "Towel Required to be Laid Down When Using Gym Equipment" just sounds like they don't want you holding a towel or that you need to lay your towel on the ground while using the equipment. It definitely needs changed.
