This medication is used to relieve moderate to severe pain from a variety of causes. Hydromorphone is a narcotic pain reliever (opiate-type). It acts on certain centers in the brain to give you pain relief.This medication should not be used to relieve pain during labor or delivery.

Uses
This medication is used to relieve moderate to severe pain from a variety of causes. Hydromorphone is a narcotic pain reliever (opiate-type). It acts on certain centers in the brain to give you pain relief.This medication should not be used to relieve pain during labor or delivery.
Notes
  • Do not share this medication with others.
  • It is against the law.This medication has been prescribed for your current condition only.
  • Do not use it later unless told to do so by your doctor.
  • A different medication may be necessary in that case.
  • Ask your pharmacist how to dispose of any unused medication.

Storage
  • Store in the refrigerator between 36-46 degrees F (2-8 degrees C) away from light.
  • Do not store in the bathroom.
  • Keep all medicines away from children and pets.

Overdose
  • Symptoms of overdose may include: slow/shallow breathing, clammy skin, limp muscles, very small pupils, loss of consciousness.

How To Use
  • Use this medication rectally, usually every 6 to 8 hours as needed or as directed by your doctor.
  • Your doctor may direct you to use this medication as needed for pain when it occurs or on a regular schedule.
  • Follow your doctor's instructions carefully.If the suppository is too soft to insert, put it in cold water before removing the wrapper.
  • Wash your hands and remove the outer wrapper of the suppository.
  • Moisten the suppository with a little water.
  • Lie down on your left side with right knee bent.
  • Gently insert the suppository into the rectum with your finger.
  • Remain lying down for a few minutes and avoid having a bowel movement for an hour or longer so the drug will be absorbed.
  • Wash your hands thoroughly after inserting the suppository.If you are using this medication as needed, it will work best if you use it as soon as the first signs of pain occur.
  • If you wait until the pain has worsened, the medication may not work as well.
  • If you are using this medication on a regular schedule, use it regularly in order to get the most benefit from it.The dosage is based on your medical condition, response to treatment, and any use of other narcotic pain relievers.If you experience nausea, consult your doctor or pharmacist about ways to decrease it (e.g., taking antihistamines, lying down for 1 to 2 hours with as little head movement as possible).This medication may cause dependence, especially if it has been used regularly for an extended time or if it has been used in high doses.
  • In such cases, withdrawal reactions (e.g., restlessness, anxiety, irritability, watering eyes, loss of appetite, nausea/vomiting/diarrhea, muscle cramps/pain/spasms, trouble sleeping, hot/cold flashes) may occur if you suddenly stop this drug.
  • To prevent withdrawal when stopping extended/regular treatment with this drug, gradually reduce the dosage as directed.
  • Consult your doctor or pharmacist for more details, and report any withdrawal reactions immediately.Rarely, abnormal drug-seeking behavior (addiction) is possible with this medication.
  • Do not increase your dose, use it more frequently, or use it for a longer time than prescribed.
  • Properly stop the medication when so directed.When used for an extended period, this medication may not work as well and may require different dosing.
  • Talk with your doctor if this medication stops working well.Tell your doctor if your pain persists or worsens.

Side Effects
  • Nausea, constipation, lack of appetite, changes in taste, blurred vision, headache, dizziness, drowsiness, dry mouth, flushing, or sweating may occur.
  • If any of these effects persist or worsen, tell your doctor or pharmacist promptly.To prevent constipation, maintain a diet adequate in fiber, drink plenty of water, and exercise.
  • If you become constipated while using this drug, consult your pharmacist for help in selecting a laxative (e.g., stimulant-type) and stool softener.Remember that your doctor has prescribed this medication because the benefit to you is greater than the risk of side effects.
  • Many people using this medication do not have serious side effects.Tell your doctor immediately if any of these unlikely but serious side effects occur: slow/shallow breathing, fast/slow heartbeat, fainting, trouble urinating, severe headaches, severe constipation, severe vomiting, severe stomach pain, shaking, mental/mood changes (e.g., anxiety, depression, hallucinations), vision problems, muscle stiffness, tingling hands/feet, weakness.Tell your doctor immediately if this rare but very serious side effect occurs: seizures.A very serious allergic reaction to this drug is unlikely, but seek immediate medical attention if it occurs.
  • Symptoms of a serious allergic reaction may include: rash, itching, swelling, severe dizziness, trouble breathing.If you notice other effects not listed above, contact your doctor or pharmacist.

Precautions
  • Before using hydromorphone, tell your doctor or pharmacist if you are allergic to it; or to other narcotic pain relievers (e.g., morphine); or if you have any other allergies.This medication should not be used if you have certain medical conditions.
  • Before using this medicine, consult your doctor or pharmacist if you have: severe breathing problems (e.g., respiratory depression, hypercapnia, severe asthma), pain from a recent scorpion sting, intoxication with medications that may slow the nervous system or your breathing (CNS/respiratory depressants such as alcohol or tranquilizers/sedatives).Before using this medication, tell your doctor or pharmacist your medical history, especially of: difficulty urinating (e.g., due to enlarged prostate, urethral stricture), personal or family history of regular use/abuse of drugs/alcohol/other substances, lung disease (e.g., asthma, hypoxia, hypercapnia, chronic obstructive pulmonary disease-COPD), gallbladder/biliary tract problems, disease of the pancreas (e.g., pancreatitis), kidney disease, liver disease, certain brain disorders (e.g., tumor, increased intracranial pressure, coma, seizures, head injury), mental/mood conditions (e.g., toxic psychosis), spinal problems (kyphoscoliosis), a severe loss of body water and minerals (dehydration), severe obesity, low blood pressure, underactive thyroid (hypothyroidism), recent surgery (including recent surgical anastomoses), stomach problems of unknown cause, adrenal gland problem (e.g., Addison's disease).This drug may make you dizzy or drowsy or cause blurred vision.
  • Use caution while driving, using machinery, or doing any activity that requires alertness or clear vision.
  • Avoid alcoholic beverages because they may increase the risk of this drug's side effects.To reduce dizziness and lightheadedness, get up slowly when rising from a sitting or lying position.Before having surgery, tell your doctor or dentist that you are using this medication.Caution is advised when using this drug in the elderly because they may be more sensitive to its side effects, especially slow/shallow breathing and drowsiness.During pregnancy, this medication should be used only when clearly needed.
  • Tell your doctor immediately if you become pregnant or think you may be pregnant.
  • If your doctor directs you to use this medication, follow your doctor's instructions closely and do not suddenly stop using this drug.
  • This medication should not be started at or near the time of delivery.
  • Infants born to mothers who have been using this medication for an extended time may have withdrawal symptoms such as irritability, abnormal/persistent crying, vomiting, or diarrhea.
  • Tell your doctor immediately if you notice any of these symptoms in your newborn.It is not known whether this drug passes into breast milk.
  • Breast-feeding is not recommended while using this drug.
  • Consult your doctor before breast-feeding.

Missed Dose
  • If you are using this medication on a regular schedule and you miss a dose, use it as soon as you remember.
  • If it is near the time of the next dose, skip the missed dose and resume your usual dosing schedule.
  • Do not double the dose to catch up.

Drug Interactions
  • Your doctor or pharmacist may already be aware of any possible drug interactions and may be monitoring you for them.
  • Do not start, stop, or change the dosage of any medicine before checking with your doctor or pharmacist first.This drug should not be used with the following medications because very serious interactions may occur: mixed opioid agonists/antagonists (e.g., buprenorphine, butorphanol, nalbuphine, pentazocine), naltrexone.Avoid taking MAO inhibitors (e.g., furazolidone, isocarboxazid, linezolid, moclobemide, phenelzine, procarbazine, rasagiline, selegiline, tranylcypromine) within 2 weeks before, during, or after treatment with this medication.
  • In some cases, a serious (possibly fatal) drug interaction may occur.If you are currently using any of these medications, tell your doctor or pharmacist before starting hydromorphone.Before using this medication, tell your doctor or pharmacist of all prescription and nonprescription/herbal products you may use, especially of: amphetamines, water pills (diuretics such as furosemide), drugs affecting liver enzymes that remove hydromorphone from your body (e.g., cimetidine, rifamycins including rifampin).Tell your doctor or pharmacist if you also take drugs that may cause drowsiness, slow your breathing, or lower your blood pressure such as: antihistamines that cause drowsiness (e.g., diphenhydramine), anti-anxiety drugs (e.g., diazepam), anti-seizure drugs (e.g., carbamazepine), medicine for sleep or anxiety (e.g., sedatives, hypnotics), muscle relaxants, other narcotic pain relievers (e.g., codeine), psychiatric medicines (e.g., phenothiazines such as chlorpromazine, or tricyclics such as amitriptyline), tranquilizers.Check the labels on all your medicines (e.g., cough-and-cold products) because they may contain ingredients that cause drowsiness.
  • Ask your pharmacist about using those products safely.This product can affect the results of certain lab tests (e.g., amylase, lipase).
  • Make sure laboratory personnel and all your doctors know you use this drug.
