Drotrecogin alfa is a man-made form of a natural enzyme in your body called activated protein C, which helps to decrease clotting and swelling (inflammation) in the blood vessels. This medication is used to improve the chance of survival in patients with severe and life-threatening infections of the blood (severe sepsis with organ failure).This drug is not recommended for use in children.

Drotrecogin alfa (Activated)

Uses
Drotrecogin alfa is a man-made form of a natural enzyme in your body called activated protein C, which helps to decrease clotting and swelling (inflammation) in the blood vessels. This medication is used to improve the chance of survival in patients with severe and life-threatening infections of the blood (severe sepsis with organ failure).This drug is not recommended for use in children.
Notes
  • Do not share this medication with others.Laboratory and/or medical tests (e.g., prothrombin time-PT) should be performed periodically to monitor your progress or check for side effects.
  • Consult your doctor for more details.

Storage
  • Store the vials of powder in a refrigerator between 36-46 degrees F (2-8 degrees C) away from light.
  • Do not freeze.Once mixed, the medication in the vial can be stored at room temperature between 68-77 degrees F (20-25 degrees C) away from heat and direct sunlight and must be used within 3 hours.
  • Throw away any unused medication.After the mixed medication is diluted with the IV fluid, the product in the infusion bag should be used at room temperature between 68-77 degrees F (20-25 degrees C) within 14 hours (within 12 hours if the product is used in a syringe pump).

Overdose
  • Symptoms of overdose may include: easy or unusual bleeding/bruising, black/bloody stool, pink/red urine, weakness on one side of the body, confusion.

How To Use
  • This medication is given by injection into a vein by a health care professional.
  • Learn all preparation and usage instructions for this product.
  • Follow all instructions for proper mixing and dilution with the correct IV fluid.
  • Before using, check this product visually for particles or discoloration.
  • If either is present, do not use the liquid.
  • Do not shake the drug when preparing, transporting, or using the medication.
  • If you have questions about the preparation or use of this medication, consult the doctor or pharmacist.Dosage is based on your weight.
  • The medication should be given for a total of 96 hours (4 days).Tell your doctor if your condition persists or worsens.

Side Effects
  • Remember that your doctor has prescribed this medication because he or she has judged that the benefit to you is greater than the risk of side effects.
  • Many people using this medication do not have serious side effects.This medication increases the risk of severe bleeding.
  • Tell your doctor immediately if any of these unlikely but serious side effects occur: easy or unusual bruising/bleeding, black/bloody stool, vomit that looks like coffee grounds, pink/red urine, severe stomach/abdominal pain.Seek immediate medical attention if any of these rare but very serious side effects occur: weakness on one side of the body, vision changes, slurred speech, confusion.A very serious allergic reaction to this drug is rare.
  • However, seek immediate medical attention if you notice any symptoms of a serious allergic reaction, including: rash, itching, swelling, severe dizziness, trouble breathing.If you notice other effects not listed above, contact the doctor.

Precautions
  • Before using drotrecogin alfa, tell your doctor or pharmacist if you are allergic to it; or if you have any other allergies.This medication should not be used if you have certain medical conditions.
  • Before using this medicine, consult your doctor or pharmacist if you have: severe bleeding problems (e.g., bleeding inside the body, severe injury), brain disorders (e.g., head injury, tumor, recent hemorrhagic stroke, cerebral herniation), recent surgery in the head or spine, an epidural catheter.Before using this medication, tell your doctor or pharmacist your medical history, especially of: low platelet counts (thrombocytopenia), certain abnormal test result (high prothrombin time-PT), stomach/intestinal bleeding, stroke, abnormal blood vessels in the brain (e.g., aneurysm), bleeding/clotting disorders, liver disease, recent surgery.Before having surgery, tell your doctor that you are using this medication.During pregnancy, this medication should be used only when clearly needed.
  • Discuss the risks and benefits with your doctor.It is not known whether this drug passes into breast milk.
  • Because of the possible risk to the infant, breast-feeding while using this drug is not recommended.
  • Consult your doctor before breast-feeding.

Missed Dose
  • If a dose is stopped or interrupted, contact the doctor to see if a new dosing schedule is needed.

Drug Interactions
  • Your doctor or pharmacist may already be aware of any possible drug interactions and may be monitoring you for them.
  • Do not start, stop, or change the dosage of any medicine before checking with your doctor or pharmacist first.Before using this medication, tell your doctor or pharmacist of all prescription and nonprescription/herbal products you may use, especially of: anti-platelet medications (e.g., clopidogrel, NSAIDs such as aspirin/ibuprofen, glycoprotein IIb/IIIa inhibitor such as eptifibatide), "blood thinners" (e.g., enoxaparin, heparin, warfarin), clot-dissolving drugs (e.g., thrombolytics such as alteplase).Do not mix drotrecogin alfa with other medications in the same IV bag or give together through the same IV line as other medications.This medication may interfere with certain laboratory tests (including activated partial thromboplastin time-APTT), possibly causing false test results.
  • Make sure laboratory personnel and all your doctors know you use this drug.
